Waiver Frontend Interface

Frontend is a web interface with a unique URL for your business that you can share with your customers. That is where your customers sign an actual Waiver form.

It consists of the following pages: 

STEP1: Customers choose the activity according to the reservation they previously made.

Example: 

If you have an Escape Room, here would be names of different adventure games you provide.

STEP 2: Then participants pick the date and time they reserved, as long as its applicable. 

Example: 

Escape Rooms operate with a schedule for each session. Thus, client choose the game, time and time session they reserved. 

This information will match their reservation in the booking system and you can easily match them  by knowing the date and time.

STEP 3: Clients get asked if there are any minors in a group and if so, a guardian is required to sign for minors.

There is a date of birth validator in the form. Form would not let a signer to process unless that person is an adult. 

Example: 

Date of birth calendar picker would not show dates applicable for current minor’s age. It would require another person (a parent or a guardian) to sign for a minor.

STEP 4: The last page represents an actual waiver form that your customers will read, agree to the terms of the contract, fill up their information along with any custom forms and place an electronic signature by using a computer mouse or finger if signing up at a kiosk station.
